Just tried your new version (July 24) for SQL server 2008. It worked much worse when I tried to format sql generated by Cognos. Is there any way I can go back the older version, please?

You can select "old style formatting" on the toolbar to use old formatter.
New formatter is very flexible but requires additional actions to configure it to suit your needs.
Currently we accumulate user feedback and will release several sample formatting templates later.

Would it be difficult to release a template that matches old style formatting that can be tweaked from there? The commas indentation is driving me crazy and I cannot seem to find the setting for indented comma, then no space, then list item for select statement lists.

We plan to release a couple of sample templates. Actually we want to improve format style editor and add preview, to make template editging easier. We also encourage users to send us templates for revision and publication.

until you release the sample templates can you tell us what needs to happen in the template we DO have to fix the annoying ", columnName" issue?

the "old" version doesn't break at the comma, so it's worse (e.g. "column1, column2, column3..."
instead of "column1
,column2
,column3"

thanks

Sorry, I do not clearly copy your question.
I suppose you ask about settings of new formatter. Please write the sample what you currently get and what you want to achieve.
